Symptom Survey Form

The Symptom Survey Form is the first step in the evaluation process, and is required for any treatment. The cost is included in the initial consultation for in-office visits. If you are submitting the form online, the cost is $39.

Hair Analysis                                                                             $225.00

Hair Analysis is a tissue biopsy that is crucial in determining how well your body is using the hormones delivered to it at the cellular level. It helps detect mineral deficiencies and determine metabolism type. Consultation included.

Adrenal Stress Index (ASI)                                                   $225.00

Adrenal Stress Index (ASI) is recommended when symptoms such as fatigue, weakness, insomnia, headaches, irritability, nervousness, sugar cravings, dizzy spells, weight gain, and decreased stamina are being experienced. ASI is also recommended for all individuals with chronic disease(s). Saliva samples are required for testing. Consultation included.

Short Post Menopause Hormone Panel (PHP1)                $200.00

Evaluates baseline DHEA, testosterone, estrone, estradiol, estriol, and progesterone. 1 saliva sample is required for testing. Consultation included.

Expanded Short Post Menopause

Hormone Panel (ePHP1)                                                        $225.00

This assessment is used for women who are experiencing menstrual irregularity, night sweats, hot flashes, vaginal dryness, depression, mental fog and forgetfulness, tearful/moodiness, and tender breasts. In addition to any of the above, adrenal symptoms would warrant using this profile. Evaluates baseline DHEA, testosterone, estrone, estradiol, estriol, progesterone, LH, and FSH. 2 saliva sample is required for testing. Consultation included. 

Female Hormone Panel (eFHP)                                      $325.00

The Female Hormone Panel is used to assess infertility, and when symptoms such as hot flashes, night sweats, insomnia, memory loss, mood swings, low sex drive, fatigue, irregular periods, breast tenderness, increased skin wrinkling, weight gain, and anxiety are present. Mapping of female cycle for estradiol and progesterone; mean testosterone and DHEA.  11 saliva sample are required for testing. Consultation included.

Hormone Panel Male - Expanded (eMHP)                            $220.00

For men with complaints of decreased libido, decreased erections, decreased stamina, decreased mental sharpness, decreased muscle size, elevated triglycerides and/or high cholesterol. Evaluates DHEA, androstenedione, testosterone, dihydrotestosterone, progesterone, estrone, LH, and FSH. 1 saliva sample is required for testing. Consultation included.

Expanded Gastrointestinal Health Panel (GI2)                    $433.00

Stool culture for yeast, ova and parasites, bacterial stool pathogens, Clostridium difficile toxins A and B, Giardia antigen, Cryptosporidium antigen, Toxoplasma gondii antibody SIgA, Ameba histolytica antibody SIgA, Helicobacter pylori antibody SIgA, chymotrypsin, occult blood, fecal pH, total intestinal SIgA, intestinal lysozyme, alpha anti-chymotrypsin, anti-gliadin SIgA, anti-dairy (casein) SIgA, anti-soy protein SIgA, and anti-egg albumin SIgA. 2 stool samples and 1 saliva sample are required for testing. Bone Health Panel (BH)                                                          $226.00

Evaluates Vitamin K (Diet Questionnaire), vitamin D, collagen cross links, DHEA, estradiol, progesterone, testosterone, and cortisol. 1 urine sample and 2 saliva samples are required for testing. Consultation included.

Iodine Loading Test                                                      $150.00

This assessment is used to determine how well the body utilizes iodine. This is particularly crucial to breast and prostate health, as well as thyroid function. It may be prescribed for one who is experiencing symptoms such as low energy, slow metabolism, hypothyroidism, fibrocystic breast, hypertension, heart palpitations, polycystic ovaries, increased breast cancer risks, diabetes, uterine fibroids. A 24-hour urine collection is required for testing. Consultation included.
